How Care Is Planned

Before support begins, we carry out an individual assessment to understand each person’s health, personal and social care needs. Care is reviewed regularly and updated whenever needs change, helping ensure support remains appropriate and responsive.

Our Approach to Care

We believe in personalised care that supports independence, respects life history and values, and focuses on what each person can do, not just what they need help with. Continuity of carers is also an important part of how we work.

Language & Communication

We recognise the importance of language, culture and communication, and we are committed to the Active Offer of Welsh language care where needed.
